Private Lesson Signup

I put together this form to collect some basic info from people who are interested in private lessons. You're probably looking at this and thinking, "Oh, great. My absolute favorite thing to do is mindlessly enter information into a web form. Can't wait..." and I completely understand. Having this information will make the process a lot easier for me though, so I would really appreciate your patience.

I promise, this form will take you a maximum of 10 minutes to fill out. It only has 4 sections, and the first one basically asks what your name is. You don't need to be too detailed; I just want to get an idea of your starting point so I know what to expect when we meet for a consultation.
